Shoulder pain generally includes shoulder joint pain, and the surrounding soft tissues. Traditional Chinese medicine (TCM) thinks the shoulder pain is due to the stagnation of Chi(oxygen) and blood, or from an attack from an external pathogen such as wind, cold, dampness, or phlegm, which can block the meridians and lead to pain and limited movement of the shoulder. TCM classifies it in “Bi syndrome”. Shoulder pain often happens due to over use, sports injury, or from a traumatic event. Since shoulder pain can be caused by different reasons, different pathogens from different environments, the course of treatment will be different for each individual.

One type of shoulder pain is due to an invasion of wind and cold. In traditional Chinese medicine, it is noted that there are 6 pathogenic factors such as wind, cold, heat, dampness, dryness and fire. This is a general term for the six climatic conditions. Human beings live in nature and have certain adaptability to various climatic variations, so the six normal natural factors cannot cause people acquire pain. However, if the climatic variations are too unusual or the individual is exposed to such conditions in excess the six natural factors will affect the human body and manifest in different ways including pain. If the shoulder is attacked by the wind and cold, the muscle around the shoulder can be achy. This type of shoulder pain is not so serious. The course of treatment is shorter, and the pain level is not very high. The character of the pain is usually dull and achy and range if motion is not much compromised. The pain may only be around the shoulder, in the soft tissue or can radiate to the scapula and upper back. If the pain is in the front of the shoulder, some discomfort may be near the neck and upper arm. Patients sometimes complain of a cold sensation in and around the joint and will have the pain relieved with the application of heat. The tongue will usually appear with a white coating. The pulse is superficial. Acupuncture with cupping is recommended.

The next type of shoulder pain is due to wind and dampness. The shoulder is not only painful, but also has heavy or distended feeling. The pain does not move or radiate. It can be uncomfortable to move the shoulder and range of motion can be limited. The muscle may be numbness or have a tingling sensation. The tongue will appear with a thin white coating. The pulse is superficial and may feel slippery. The treatment suggested is to use acupuncture and TuiNa(Chinese Massage).

The shoulder can also be invaded with pain due to retention of phlegm and dampness. Retention of phlegm, in traditional Chinese medicine, is a pathological product in the body, which can be non substantial and formless, formed from the body itself or from exposure to external pathogenic factors. It can cause onset of pain. This kind of shoulder pain is more serious. The shoulder pain is usually more chronic and the treatment protocol lasts a bit longer. The range of motion can be slightly limited depending on the amount of inflammation, certain movements can lead to more pain. Over long a long period of time without therapy or some form of treatment the shoulder joint can become locked. The patient may have cold sensation around the shoulder. The heat can temporarily reduce the pain. Application of ice can make the pain worse, causing decrease in blood flow and circulation. After application of ice the pain will diminish for a brief period of time however, the pain and cold sensation will be the same if not worse as once the ice is removed. The ice restricts blood flow and circulation to the tendons, so decreasing inflammation momentarily, but returning pain the same or worse after removal. Since the pain level is high it can interfere with sleep, digestions and normal working routine. The patients commonly present with the deficiency of energy, such as, spontaneous sweating, short of breath, easily tired, or frequently catching a cold etc. The tongue is pale red, covers with thick white coating. The pulse is wiry and smooth, or wiry and thready. The treatment is advised to use acupuncture, cupping, TuiNa and Chinese herbs and special exercise/stretching for shoulder function.

The last kind of shoulder pain is due to blood stasis. This kind of shoulder pain comes from trauma, such as twisting, falling down, or accident etc. The pain may be severe or feel like a twinge. The tongue is usually red and purple or with purple spots. The pulse is thready and uneven. If the shoulder pain is not treated in a timely manner, no matter what kinds of pain it is, it will eventually become blood stasis shoulder pain. If the pain results from injury, the pain may combine with tenderness, swollen and distended. Both of them have symptoms of frozen shoulder. The treatment should be to use acupuncture, cupping,TuiNa and Chinese herbs to remove blood stasis, stimulate blood circulation and resolve pain.
